JOHNSON CITY, Tenn. (AP) -- A licensed practical nurse is charged in Johnson City with mentally and physically abusing an adult patient at a home for mentally retarded persons.

Katrina D. Frizzell, of Elizabethton, was arrested Monday on an indictment returned by a grand jury last week.

The 54-year-old Frizzell is charged with one count of willful abuse and neglect or exploitation of an adult.

She worked for Omni Visions, which is based in Nashville and operates residential facilities.

The Tennessee Division of Mental Retardation Services is also investigating after Omni Visions failed to report the allegations within four hours of discovering them, as required.
